Make Running Fun Cross Country Camp Check List

Please return all the following paperwork fully completed by AUGUST 1.  Use the checklist below to ensure all the paperwork is complete.  All paperwork must be fully completed and returned with your balance for your athlete to be fully registered for camp.

- Balance made out to North Jersey Running Association

-Emergency Contact Form

- Health History filled out and signed by parent or guardian

- Physical Form filled out, signed, and dated by a doctor (The date of the physical must be after August 20, 2022).

Release Form filled out and signed by both parent/guardian and athlete.

- Copy of Insurance Card, front and back, or insurance information you may have.

If your athlete has already received a physical for the upcoming season (at school or with your doctor) you can send us a copy of that physical.  Be sure to include the health history portion filled out and signed by the parent/guardian.  The physical must be signed by a doctor and dated after August 22, one year prior to camp.

Please return ALL of the above fully completed by AUGUST 1.

CAMP REGISTRATION

PAYMENT

  • $645.00 - Includes all meals, lodging in cabins, all instruction, lectures, and complete 24-hour adult supervision.

  • Send a $75.00 Deposit with application. (Non-Refundable)

  • Checks, cash, or money orders should be made out to North Jersey Running Association and sent to 310 Hamilton Ave Hasbrouck Heights, NJ 07604. Deposits should be received on or before June 15. Balance is due by July 20.
